Concierge Medicine vs. Traditional Primary Care: What’s Different

Patients ask us regularly: do I need concierge medicine if I already have a primary care doctor? It is a fair question, and the honest answer depends on what is actually frustrating you about your current care. This post compares the two models on the specific friction points that drive patients into concierge programs, and is candid about when concierge is overkill.

What Traditional PCP Care Does Well

A good traditional primary care relationship covers the basics of adult medicine: annual physical, screening tests appropriate for your age, immunization schedule, management of stable chronic conditions, and referrals to specialists when needed. Most adults can have a productive PCP relationship for decades.

Strengths of traditional PCP care:

  • Insurance-covered for most patients with low or no out-of-pocket annual cost
  • Established cadence of preventive care
  • Familiar relationship with one physician over years
  • Integrated into established health systems and referral networks

Where Traditional PCP Care Struggles

The friction points patients raise to us most often:

  • Six to twelve week wait for an appointment
  • Fifteen minute appointments that feel rushed
  • Limited access between visits: messages routed through staff, slow callbacks
  • After-hours questions routed to an answering service
  • Limited coordination when traveling
  • No house call option for sick visits
  • Insurance billing constraints driving short visits and limited preventive depth
  • PCP changes when the practice or insurance network shifts

Most of these are systemic problems, not failures of individual physicians. The traditional PCP model in the US is squeezed by insurance reimbursement structures that demand high patient panels and short visits.

What Concierge Medicine Adds

Concierge medicine solves the access and depth problems by capping the patient panel and replacing insurance billing with membership fees. The specifics vary by program, but the consistent elements:

  • Direct phone, text, or app access to your physician
  • Same-day or next-day appointments
  • Longer visits (45 to 90 minutes is typical for annual workup)
  • More-detailed preventive care including comprehensive lab panels
  • After-hours physician availability for true clinical questions
  • Travel medical support
  • Coordination with specialists and pharmacies
  • House calls in some programs (TrufaMED includes house calls across all tiers)
  • Hospital escort and ER advocacy in some programs (TrufaMED’s Signature tier includes this)

When Concierge Adds Real Value

Concierge is most valuable for adults in these categories:

  • Anyone whose time is genuinely constrained and the wait/coordination friction of traditional PCP care is a real cost
  • Adults with complex medical history requiring coordination across multiple specialists
  • Frequent travelers, including international
  • High-net-worth households where the cost of the membership is small relative to the value of access
  • Adults preparing for or recovering from significant surgery
  • Caregivers managing elderly parents or chronically ill family members (the family-tier coverage is valuable)
  • Adults with anxiety about medical access who benefit from knowing they can call

When Concierge Is Overkill

Concierge is not the right fit for every adult:

  • If your current PCP is responsive, available, and meeting your needs, no upgrade is needed
  • If your medical history is uncomplicated and you rarely need clinical visits, the membership cost may not justify the benefit
  • If insurance coverage is your primary access mechanism and you cannot or do not want to add an out-of-pocket cost
  • If you want longitudinal multi-decade primary care: for the established screening cadence, a traditional PCP relationship is the right tool. Concierge layers on top, not as replacement

How TrufaMED’s Concierge Fits Alongside Your PCP

We coordinate alongside your existing primary care physician. We handle acute issues, urgent questions, house calls, hospital escorts, travel coverage, and detailed preventive workup. Your PCP keeps the multi-decade screening cadence and the established relationship. We exchange notes and lab work directly so the two visits do not duplicate.

If you do not have a PCP, we can help you establish one in Miami while you are a concierge member. We have referral relationships across the city’s primary care community.

Frequently Asked Questions

Do I need to drop my PCP to join concierge?

No. Most of our concierge members keep their existing PCP. We coordinate alongside, not in place of, their PCP relationship.

What if my PCP and concierge physician disagree?

We coordinate directly. Disagreements are uncommon; when they happen we discuss with you transparently and reach a plan you understand.

Does insurance still apply for visits at TrufaMED if I’m a concierge member?

For services where insurance applies (urgent care visits with covered diagnoses, certain labs, vaccinations) we bill insurance. The concierge membership is the layer on top of any insurance-covered service.

Can I try concierge for one year and decide?

Yes. Memberships are annual. There is no multi-year commitment required.
