Financial Agreement & Payment Policy

Payment Terms

Payment is due at the time of service. TrufaMED accepts cash, credit card, debit card, and HSA/FSA cards. By providing a payment card, you authorize TrufaMED to charge that card for all services rendered, including any patient-responsibility balance remaining after insurance adjudication (deductibles, copayments, coinsurance, and non-covered services).

Insurance

TrufaMED will submit claims to participating insurance carriers on your behalf as a courtesy. You represent that all insurance information you provide is accurate and current. You remain personally and fully responsible for all charges not covered by your insurance. Verification of benefits is not a guarantee of payment or coverage. You assign to TrufaMED all insurance benefits payable for services rendered.

Self-Pay

If you are uninsured or elect to pay out-of-pocket, full payment at self-pay rates is due at the time of service.

Responsible Party

The adult presenting a minor for treatment is the financially responsible party and is personally liable for all charges, regardless of custody arrangements, divorce decrees, or third-party insurance obligations.

Returned Payments & Collections

Returned checks or declined payments are subject to a 5 fee. Delinquent accounts may be referred to collections, and you are responsible for all reasonable costs of collection, including attorney’s fees and interest at the Florida statutory rate.

No Refund Policy

All payments for services authorized and received at TrufaMED are final and non-refundable. By signing the Patient Registration and Financial Responsibility Acknowledgment and presenting for treatment, you authorize the charge and acknowledge receipt of services at the conclusion of your visit. Dissatisfaction with clinical outcome, subsequent insurance determination, or any other post-service circumstance does not constitute non-receipt of services and does not entitle you to a refund or chargeback.

Chargebacks

To resolve any billing concern, you agree to contact TrufaMED directly at (305) 537-6396 before initiating any dispute with your financial institution. Initiating a chargeback for services you authorized and received constitutes a material breach of this agreement. TrufaMED reserves the right to recover the full disputed amount, all chargeback and processing fees, attorney’s fees, costs, and interest, and to refer the account to collections. Patients who initiate wrongful chargebacks may be discharged from the practice and barred from future services.

Venue

Any dispute arising out of or related to this agreement or services rendered shall be brought exclusively in the state courts of Miami-Dade County, Florida.
