The right time to run a comprehensive longevity workup is when you have a clean baseline. Late September through early November is the sweet spot for most Miami adults: summer training and travel are recent enough to inform results, holiday season has not yet introduced caloric and stress confounders, and you have time to act on findings before year-end.
This is the panel we run annually for our preventive care and concierge patients pursuing serious longevity goals.
The Core Panel
Cardiovascular risk
- Lipid panel: total cholesterol, LDL-C, HDL-C, triglycerides
- ApoB: the most informative single cardiovascular risk marker
- Lp(a): genetic risk factor, one-time test typically
- hsCRP: inflammation marker
- Homocysteine
- Optional: lipoprotein particle analysis (NMR) for advanced lipidology cases
Metabolic
- Fasting glucose
- HbA1c
- Fasting insulin (often more informative than HbA1c in pre-diabetic patients)
- Comprehensive metabolic panel: kidney function, liver function, electrolytes
- Uric acid
Hormones
- Total and free testosterone (men) or full female panel (estradiol, progesterone, FSH, LH, DHEA-S) with cycle context
- SHBG
- Full thyroid: TSH, free T4, free T3, reverse T3, anti-TPO, anti-TG
- Cortisol AM (fasted)
- IGF-1
- DHEA-S
- Prolactin
Vitamins and minerals
- Vitamin D 25-OH
- Vitamin B12, folate, methylmalonic acid
- Ferritin and iron panel
- Magnesium RBC
- Zinc
- Omega-3 index (optional)
Hematology and inflammation
- Complete blood count with differential
- Erythrocyte sedimentation rate
- Fibrinogen
Cancer screening (age-appropriate)
- PSA in men over 40 or with family history
- CA-125 in women based on individual risk
- Colonoscopy on appropriate cadence (typically every 10 years starting age 45)
- Mammography annually starting age 40
- Skin exam annually with dermatology
- Optional: multi-cancer early detection test (Galleri or equivalent) for high-risk patients
Imaging (selected)
- Coronary artery calcium score: one-time scan, valuable for patients without established cardiovascular risk
- DEXA scan: bone density, body composition; annually or every other year
- Full-body MRI: optional, for high-risk patients with longevity-focused screening goals
- Echocardiogram if clinically indicated
How We Run It
The workup is delivered as a two-visit sequence for most patients:
- Visit 1 (45 to 60 minutes): History review, goals discussion, physical examination, blood draw. We send out specialized tests with results returning in 7 to 14 days.
- Visit 2 (45 to 60 minutes): Results review, individualized recommendations, supplement protocol, any prescription medications, and follow-up cadence agreed.
Concierge members can do this in-clinic or as a home visit; preventive care patients typically do it in-clinic.
Acting On Findings
The panel is only as useful as the actions it informs. Common outcomes:
- Vitamin D repletion if 25-OH-D is below 40 ng/mL
- Iron repletion if ferritin is below 50 ng/mL (with consideration of why)
- Thyroid optimization if TSH and free T3 patterns suggest sub-clinical dysfunction
- Lipid management if ApoB is above target for individual risk
- Cardiac referral if coronary calcium score is elevated or new murmur on exam
- Hormone optimization where clinically indicated and patient interested
- Inflammation workup if hsCRP is persistently elevated
- Sleep evaluation if cortisol and metabolic patterns suggest disrupted sleep
Cost
Out-of-pocket: $700 to $1,800 depending on which add-ons are included. Significant portions are covered by insurance when clinically indicated. We provide itemized invoices for reimbursement.
Concierge membership often makes the annual workup more straightforward; some tier benefits include the panel as part of the membership.
Frequently Asked Questions
Should I fast for the blood draw?
Yes: 10 to 12 hour fast. Water is fine. We schedule morning blood draws.
How long do specialized results take?
Most standard results come back same-day to 24 hours. Specialty markers (ApoB, Lp(a), specialty hormones) take 5 to 14 days.
Do I need to repeat this every year?
Core metabolic, hormones, vitamins, and inflammation: annually. Cardiovascular calcium score: typically one-time or every 5 to 10 years. Cancer screening: per age-appropriate schedule.
