Miami Beach private schools (Cushman, Carrollton, Lehrman, Riviera, Beach Day, Lighthouse, Hebrew Academy) and Miami-Dade public schools all require physical exam forms before the school year. Requirements vary by school and grade. This post is the guide for the most common forms parents bring us in mid-August.
The DH 3040 / 680 Forms
Florida public schools require the DH 680 immunization form for kindergarten and seventh grade entry. Some schools require a complete physical exam (DH 3040) annually for athletes or PE participants. Private schools often have their own internal forms.
At our clinic we complete the school-specific form whatever it asks for, including:
- Immunization records (we add the new ones if needed)
- Vision screening
- Hearing screening if requested
- Vital signs and growth percentiles
- Cardiac auscultation
- Musculoskeletal screen
- Standard well-child review of medical history
- Mental health and behavioral screen for older students
Required Vaccinations By Grade
Florida public school entry vaccination requirements as of 2026:
- Kindergarten: DTaP (5 doses), polio (4 doses), MMR (2 doses), varicella (2 doses), Hep B (3 doses), Hep A (2 doses)
- Seventh grade: Tdap booster, second MMR if needed
- Recommended (not required): HPV (starting at age 9-11), meningococcal at age 11-12, flu annually
Private schools generally follow the same schedule. We carry all routine pediatric vaccines on site and can administer at the physical visit.
What To Bring
- School-issued physical form (or download from school website)
- Up-to-date immunization records
- Insurance card
- List of any medications, allergies, prior surgeries or hospitalizations
- Glasses or contacts if used
Scheduling
Walk-in or scheduled. Annual school physicals are covered as preventive care under most insurance plans with no copay. Self-pay rates start at $89 for the basic physical.
We see kids of all ages, from infants through high school seniors. Mid-August is our peak school-physical season; book online to skip the wait.
Special Concerns
Bring up at the visit:
- Significant family history of cardiac disease, sudden death, blood clots, or genetic conditions
- Any prior concussion or significant head injury
- Ongoing mental health concerns
- Vision or hearing changes since last year
- Significant changes in appetite, sleep, energy, or mood
- Any new chronic condition diagnosis since last physical
- Bullying or social concerns: the physical is one of the few private settings where students can raise these to a physician
Frequently Asked Questions
Can my child get vaccines and the physical at the same visit?
Yes. We typically combine. Multiple vaccines per visit are routine and well-tolerated.
What if my child is behind on vaccines?
We catch up using the CDC catch-up schedule. Most missed vaccines can be caught up over 2 to 4 visits depending on which are missing.
Do you take all major insurance for school physicals?
Yes: Aetna, Cigna, Humana, UnitedHealthcare, and most travel/student insurance.
Can I get the form same-day?
Yes. The completed form is given to you at the end of the visit.
