September is National Suicide Prevention Awareness Month. It is also the time of year when back-to-school stress, post-summer reset, and the slow build to fall holidays predictably increase mental health visits. This post is the concierge member’s guide to mental health access through TrufaMED and a public-facing resource list for anyone reading this who needs help now.

If You Or Someone You Know Is In Crisis Right Now

  • 988 Suicide and Crisis Lifeline: call or text 988 anytime
  • Crisis Text Line: text HOME to 741741
  • If immediate safety is at risk: call 911 or go to the nearest emergency room (Mount Sinai Miami Beach, HCA Aventura, Jackson Memorial)
  • TrufaMED concierge members: call your dedicated line. After-hours physician on call.

What Concierge Mental Health Access Looks Like At TrufaMED

We are not a psychiatric or behavioral health specialty practice. We are a primary care, urgent care, and concierge medicine practice with established referral relationships and the ability to coordinate access across the Miami mental health system. For concierge members, this matters because we shorten the time-to-evaluation.

What we do internally:

  • Initial evaluation for mood, anxiety, sleep, and stress concerns
  • Routine SSRI/SNRI management for stable patients with established treatment plans
  • Coordination with your existing psychiatrist or therapist
  • Sleep evaluation and management
  • Substance use screening and bridging care to specialists
  • Crisis triage with same-day specialist referral

What we refer out:

  • Initial diagnostic psychiatric evaluation for complex presentations
  • Long-term psychotherapy
  • Inpatient and intensive outpatient programs
  • Specialized care: eating disorders, OCD, PTSD, complex trauma, ADHD evaluation
  • Substance use treatment programs

We have established relationships with several Miami psychiatrists, therapists, and intensive outpatient programs and can coordinate referrals with shorter wait times than self-referral typically achieves.

What To Do If You Or A Family Member Is Struggling

First step: tell someone. The barrier to mental health care is almost always the first conversation. Your primary care physician, your concierge physician, a trusted friend, or our urgent care line is a starting point. We are unsurprised when patients raise mental health concerns at routine visits; it is a substantial portion of what we manage.

Practical steps:

  • Schedule a visit if you have been struggling for more than two to three weeks, particularly with sleep, appetite, energy, motivation, or interest in usual activities
  • Reach out for a same-day evaluation if symptoms are severe, if there are thoughts of self-harm, or if function at work or home is compromised
  • Bring a partner or family member to the visit if it helps you describe what is happening
  • Be specific with your physician: what changed, when, what has worked or not worked in the past

Sleep As The Underlying Factor

We say this constantly because it is true: sleep is the single most under-addressed factor in adult mood and anxiety presentations. Chronic insomnia, untreated sleep apnea, shift work, and inconsistent sleep schedules cause and worsen depression and anxiety more reliably than any single other factor. If your sleep is broken, the foundation is missing.

Our preventive care visits include a sleep evaluation. Sleep study referral is straightforward when indicated.

Mental Health In Concierge Care: What’s Different

Two real differences for concierge members:

  • Same-day or next-day evaluation when a patient calls saying something has changed. This matters because mental health emergencies do not respect business hours.
  • Continuity. The same physician knows the patient’s history, family situation, recent life stressors, medication tolerability profile, and what has and has not worked before. This is materially different from cycling through urgent care or new psychiatrists.

Frequently Asked Questions

Do you prescribe SSRIs at urgent care?

We prescribe maintenance medication for stable patients with established treatment plans. We do not initiate complex psychiatric medication management at a single urgent care visit; we refer to psychiatry for that.

Can I get a same-day mental health visit?

Yes for concierge members. For non-members, our urgent care line can triage and arrange evaluation within 24-72 hours typically.

Do you offer therapy?

No, we do not have on-staff therapists. We refer to vetted Miami therapy practices for cognitive behavioral therapy, EMDR, family therapy, and other modalities.

Will my visit be confidential?

Yes, protected by HIPAA. Insurance billing creates a record with your insurer for claims processed; cash-pay visits do not generate insurance records.
