Tim Ferriss has been more transparent than almost any public figure about his own self-experimentation: lab panels, training cycles, supplements, peptides, recovery interventions, and the cost of every intervention. His writing and podcasts have shaped a generation of biohackers. Patients ask us regularly: how do I build a Tim Ferriss-style protocol responsibly?
This post walks through the public elements of his approach, the elements we recommend, the elements that need physician supervision in 2026, and a sensible order to build a stack without overdoing it.
The Public Ferriss Approach — Recurring Themes
Across decades of podcasts, his book Tools of Titans, his blog, and his own writing, several themes recur:
- Quantification before intervention — comprehensive lab work as the baseline
- Targeted hormone optimization where clinically supported
- Selected peptides for recovery and metabolic effect
- Aggressive recovery: sauna, cold exposure, sleep optimization
- Slow carb / time-restricted eating with periodic refeeds
- Resistance training fundamentals plus martial arts
- Periodic medical workups, including advanced imaging in some cases
Step 1: The Lab Panel
The first move in any Ferriss-style protocol is the comprehensive lab panel. We refer to this as the longevity baseline and run it at our Surfside clinic. The standard panel includes:
- Comprehensive metabolic panel, lipid panel, advanced lipid (ApoB, Lp(a)), hsCRP, homocysteine
- Full thyroid panel: TSH, free T4, free T3, reverse T3, anti-TPO, anti-TG
- Hormones: total and free testosterone, SHBG, estradiol (sensitive), DHEA-S, cortisol AM, prolactin, IGF-1
- Hematology: CBC with differential, ferritin, iron panel
- Metabolic: fasting insulin, HbA1c, fasting glucose, OGTT if indicated
- Vitamin panel: 25-OH vitamin D, B12, folate, magnesium RBC
- Inflammation and immune: complete autoimmune panel where indicated
- Coagulation: optional baseline INR/PT/aPTT for travelers
- PSA (in older male patients)
- Optional: omega-3 index, sex hormone binding globulin, full lipoprotein particle analysis

Step 2: Foundations Before Optimization
Ferriss is consistent on this: foundations come first. No peptide, no IV, no hormone protocol will compensate for chronically inadequate sleep, no resistance training, or terrible diet.
The foundations:
- Sleep 7 to 8.5 hours nightly with consistent timing — tracked via wearable
- Resistance training 3 to 4 sessions weekly with progressive overload
- Cardio mix: zone 2 endurance work, with selected high-intensity intervals
- Diet: real food, adequate protein (~1 g/lb lean mass), minimum processed food
- Sauna 3 to 4 sessions weekly, cold exposure 2 to 4 sessions weekly once tolerated
- Stress management: meditation, breathwork, time outdoors
Step 3: TRT Where Indicated
Ferriss has been public about his use of testosterone replacement therapy in some periods. As we wrote in our Joe Rogan biohacker stack post, TRT requires baseline labs, clinical indication, conservative dosing, and ongoing monitoring. Same principles apply here.
Step 4: Peptides — The Current Landscape
Peptide regulations have tightened significantly. Sermorelin and tesamorelin are FDA-approved growth hormone–releasing peptides we prescribe for appropriate indications. Step 5: IV And HBOT For Recovery
Periodic IV therapy and HBOT are reasonable additions for high-performing adults focused on recovery. Common cadences:
- Myers’ Cocktail or Immune Boost — monthly during heavy training cycles or travel-heavy quarters
- NAD+ — quarterly or before high-stress periods
- HBOT — 1.5 ATA / 60-minute sessions, 1 to 2 per week for active training periods; 3 to 5 sessions concentrated for acute recovery from injury or surgery
Step 6: Specialized Imaging Where Reasonable
Ferriss has discussed periodic specialized imaging (DEXA scans, full-body MRI, calcium score) as part of his longevity routine. These have meaningful utility for adults over 40 with longevity goals. We can coordinate referrals through our concierge program.
