Pre-Visit Intake (Online, ~15 Minutes)

Before your appointment, you complete an online intake that covers personal medical history, family history, lifestyle questionnaire (sleep, stress, nutrition, exercise, substance use), full medication and supplement reconciliation, and any wearable data you choose to share (Apple Health, Oura, WHOOP, Garmin). Intake & Vitals (15 Minutes On-Site)
