Sinus infections are one of the most common reasons people visit urgent care — and one of the most common conditions people try to treat at home for too long. While mild sinus congestion often resolves on its own, bacterial sinusitis requires medical treatment to prevent complications.

Viral sinus infections typically peak around days 3-5 and improve by day 10. If your congestion, facial pressure, and thick nasal discharge have persisted beyond 10 days without improvement, you likely have a bacterial infection that requires antibiotics. The longer you wait, the harder it becomes to clear.
A low-grade fever is common with viral illness. But a temperature above 102°F alongside sinus symptoms suggests a bacterial infection or a secondary complication. High fever with facial pain warrants same-day medical evaluation — not another round of over-the-counter decongestants.
This pattern — improvement followed by a sharp decline — is called "double worsening" and is one of the clearest indicators of bacterial sinusitis. It means a bacterial infection has developed on top of what started as a viral cold. This pattern almost always requires prescription treatment.
Intense pain concentrated around your eyes, forehead, or cheeks — especially if only on one side — suggests a significant sinus infection. If you notice visible swelling around your eyes or forehead, seek care immediately as this can indicate the infection is spreading beyond the sinus cavities.
If decongestants, saline rinses, and pain relievers are no longer providing any relief, your body is telling you the infection has progressed beyond what home treatment can address.
