When should you see a doctor for a sinus infection?
A sinus infection happens when the cavities around your nose become inflamed and swollen, trapping mucus and creating pressure. Most are viral and ease within seven to ten days. See a physician at TrufaMED if symptoms last longer than ten days without improving, if a fever climbs above 102F, if facial pain or swelling is severe, or if you start to feel better and then suddenly worse. Those are the signs a bacterial infection may need treatment. Walk in seven days a week, no appointment needed.
What a sinus infection feels like
Sinusitis is more than a runny nose. It centers on pressure and fullness in the face that drags on longer than a cold, often with these symptoms together.
Facial pressure and pain
A deep ache or fullness across the cheeks, brow, and around the eyes, often worse when you bend forward or lie down. The hallmark of sinusitis.
Congestion and blockage
A stuffed, blocked nose that does not clear, making it hard to breathe through one or both sides for days at a time.
Thick nasal drainage
Discolored mucus from the nose or down the back of the throat. Thick yellow or green drainage alone does not prove a bacterial cause.
Headache and tooth pain
Pressure-driven headache across the forehead, plus aching in the upper teeth and jaw when the sinuses behind the cheeks are involved.
Reduced sense of smell
A dulled sense of smell and taste while the sinuses are swollen, usually returning as the inflammation settles.
Fatigue, cough, and fever
Tiredness, a lingering cough that is often worse at night, and sometimes a low-grade fever as your body fights the infection.
Viral or bacterial? It changes everything
Nearly nine in ten acute sinus infections are viral, and antibiotics do nothing for a virus. The whole point of a physician visit is to tell the two apart, so you get the treatment that actually helps and skip the one that does not.
Viral sinusitis · the common one
- Usually follows a cold and is the large majority of cases
- Symptoms peak early, then steadily improve within seven to ten days
- Treated with saline rinses, decongestants or a nasal steroid spray, hydration, and rest
- Antibiotics do not help and are not prescribed
Bacterial sinusitis · less common
- Symptoms last beyond ten days without improving
- You start to get better, then suddenly worsen
- Severe from the start, with a high fever and intense facial pain
- A targeted antibiotic is appropriate, prescribed on-site
Color of mucus alone does not decide it. Thick yellow or green drainage is common in both, so duration and pattern matter far more than appearance.
We do not hand out antibiotics by default
It is easy to leave a clinic with an antibiotic prescription for a sinus infection. It is also, most of the time, the wrong call. Because the large majority of sinus infections are viral, an antibiotic usually adds side effects without shortening the illness, and overuse fuels antibiotic resistance that makes these drugs less effective when you truly need them.

A physician examines you, weighs how long your symptoms have lasted and how they have changed, and reserves antibiotics for sinusitis that is genuinely bacterial. What your sinus visit includes

On-site testing
Rapid strep and flu tests to rule out overlapping conditions, plus additional lab work if your physician suspects a secondary infection.
Targeted treatment
Decongestants, nasal corticosteroids, and pain relief as appropriate, and a targeted antibiotic only when bacterial sinusitis is confirmed.
Imaging if needed
On-site digital X-ray for recurrent sinusitis or suspected complications, with immediate results and no separate radiology appointment.
Follow-up plan
Clear discharge instructions, medication guidance, and recovery milestones, with telehealth follow-up available if symptoms persist.
Acute, subacute, and chronic sinus infections
Sinusitis affects more than 30 million adults in the United States every year, which makes it one of the most common reasons people seek same-day care. It is grouped by how long it lasts, and each type calls for a different approach.
Acute sinusitis (under 4 weeks)
The most common form, usually triggered by a cold and most often viral. It tends to resolve with supportive care, and the key decision is simply whether the small share of bacterial cases needs an antibiotic.
Subacute sinusitis (4 to 12 weeks)
An infection that lingers past the usual window. It deserves a closer look at contributing factors such as allergies, lingering inflammation, or an incomplete recovery from an earlier episode.
Chronic and recurrent sinusitis
Symptoms beyond twelve weeks, or several episodes a year, point to an underlying driver rather than a single infection. Allergies are a frequent culprit, so we may recommend allergy evaluation, and we coordinate referral to an ENT specialist when chronic sinus disease or a structural issue is likely. The aim is to break the cycle, not just treat today’s episode.
Warning signs that need the ER
A sinus infection spreading beyond the sinuses is rare, but it is serious. If you have any of these, go to the nearest emergency room or call 911 rather than waiting for a walk-in visit.
Vision changes
Double vision, blurred vision, or trouble moving an eye can signal that infection has reached the area around the eye. Seek emergency care right away.
Swelling around an eye
Redness, swelling, or pain around or behind an eye, especially with fever, needs emergency evaluation, not a walk-in appointment.
Severe headache with stiff neck
A severe, sudden headache with a stiff neck, confusion, or sensitivity to light needs immediate emergency assessment.
High fever with facial swelling
A high fever paired with significant swelling of the forehead, cheek, or around the eyes warrants emergency care without delay.

How do I know if my sinus infection is bacterial or viral?
Most sinus infections are viral and clear on their own within seven to ten days. A bacterial infection is more likely when symptoms last longer than ten days without improving, when they start to get better and then suddenly worsen, or when you have a high fever (above 102F) along with thick, discolored drainage and severe facial pain. The distinction matters because antibiotics only help bacterial infections. Our physicians examine your sinuses, review how long symptoms have lasted, and decide whether antibiotics are actually warranted, rather than prescribing them by default.
Do I need antibiotics for a sinus infection?
Usually not. Roughly nine in ten acute sinus infections are viral, and antibiotics do nothing for a virus. Prescribing them anyway only adds side effects and drives antibiotic resistance. We follow the same stewardship standard as a Joint Commission-accredited facility: antibiotics are reserved for sinusitis that is clearly bacterial, meaning symptoms persisting beyond ten days, worsening after an initial improvement, or severe from the start. When is a sinus infection an emergency?
Go to the emergency room or call 911 if you have vision changes or double vision, swelling or redness around an eye, a severe headache with a stiff neck, confusion, or high fever with significant facial swelling. These can signal that an infection has spread beyond the sinuses, which is rare but serious. For ordinary sinus pressure, congestion, and drainage, walk-in urgent care is the right and far less expensive choice.
Do I need an appointment, or can I walk in?

How can I relieve sinus pressure and congestion at home?
For viral sinusitis, the goal is comfort while your body clears the infection. Saline nasal rinses, steam, staying well hydrated, an over-the-counter decongestant or nasal corticosteroid spray, and rest all help. A warm compress over the cheeks and forehead eases facial pressure. If you are not improving after ten days, or you feel worse instead of better, that is the signal to be seen, and a physician can confirm whether something more is going on.
Can TrufaMED treat my child’s sinus infection?
Yes. We provide pediatric urgent care for children and adolescents, and our physicians are experienced in evaluating sinusitis at every age. Children show sinus infections a little differently, often with a lingering cough, daytime congestion, and irritability rather than classic facial pain, so the same viral-versus-bacterial judgment applies before any antibiotic is considered.
My sinus infections keep coming back. What can be done?
Recurrent or chronic sinusitis (symptoms lasting beyond twelve weeks, or several episodes a year) deserves a closer look than a single antibiotic course. On-site digital X-ray can help assess the sinuses, and allergies are a common hidden driver, so we may recommend allergy evaluation to find and treat the underlying trigger. When a structural problem or chronic sinus disease is likely, we coordinate referral to an ENT specialist. The aim is to stop the cycle, not just treat this episode.
Is it a sinus infection, a cold, or allergies?
They overlap, which is why an exam helps. A cold usually brings sneezing and a sore throat and fades within a week. Allergies tend to cause clear drainage, itchy or watery eyes, and a seasonal or exposure pattern, and they respond to antihistamines. A sinus infection centers on facial pressure and fullness, thicker drainage, and symptoms that drag on past ten days. Our physicians sort out which one you have so the treatment actually fits, whether that is allergy care, supportive treatment, or antibiotics.
