See your results in the TrufaMED app, or request a full copy online. We send records within 5 business days.

Your records in the TrufaMED app

Once your account is linked, lab results, urgent care records and imaging reports appear in the app as soon as our clinical team releases them.

Download the app

TrufaMED is free on the App Store and Google Play. Create an account with the email you gave us at your visit.

Link your records

Ask the front desk for a 6-digit code at your visit, then enter it in the Records tab. Your results appear there from then on.

Parents and caregivers

A parent can see a child’s records from their own app until the child turns 18. With a signed authorization, an adult can give someone they trust the same access. Our front desk sets this up in person.

Request a copy online

Use our secure form to request your own records, or your child’s records if they are under 18. We match the request to your chart, then email you a secure link within 5 business days.

The link opens after you enter the patient’s date of birth and stays available for 30 days, so you have time to download and save your copy.

What we need to verify your request

Every request is checked by our team before anything is released.

Name and date of birth

The patient’s full name and date of birth, exactly as they appear on your visit paperwork.

Email and phone on file

Use the email and phone number you gave us at your visit. If they have changed, we will call to confirm before we send anything.

Records for someone else

A parent or legal guardian can request records for a patient under 18, and we confirm the relationship before release. For another adult’s records, we need that patient’s signed Authorization for Release of Health Information.

Returning a form

Fax the signed form to (305) 686-1965, email it to [email protected], or bring it to 9445 Harding Ave in Surfside. We send records within 5 business days of receiving a complete form.

Sending records to a doctor or a third party

If another physician is treating you, their office can fax a records request to (305) 686-1965 and we will send what they need for your care. For anyone else, such as an attorney, an employer or an insurance company, we need your signed Authorization for Release of Health Information naming who receives the records and why.

Older records

The app shows the results our clinical team has released to it. For earlier visits, complete visit notes, or anything you do not see in the app, request a copy online or with the Medical Records Request form.

Medical records questions

How long does a records request take?

Up to 5 business days from the day we receive a complete request. Results released to the TrufaMED app are available there as soon as our clinical team releases them.

Why does the link ask for a date of birth?

The secure link opens only after the patient’s date of birth is entered, so your records stay private if the email is forwarded. Each link works for 30 days.

Can I request my child’s records?

Yes, for a child under 18, as the parent or legal guardian. Use the online form or the paper Medical Records Request, and we will confirm the relationship before release.

Can my doctor get my records directly?

Yes. Your physician’s office can fax a request to (305) 686-1965. To send records anywhere else, sign the Authorization for Release of Health Information.
