Rapid strep tests are highly accurate but carry a small false-negative rate, particularly in symptomatic adults. When your physician has high clinical suspicion of strep despite a negative rapid test, we send a throat culture to a certified laboratory for confirmation. Culture results typically return within 24–48 hours, and we will contact you directly if treatment is indicated.

When your rapid strep test is positive, your physician prescribes an antibiotic before you leave — no second visit, no telehealth delay, no sent-to-pharmacy-and-wait.

First-Line Antibiotics for Strep Throat

Strep throat responds rapidly and completely to antibiotics. The choice of antibiotic depends on your age, allergies, and prior treatment history. TrufaMED physicians prescribe from evidence-based, guideline-consistent options including:

  • Amoxicillin — the standard first-line choice for most patients, highly effective against Group A Strep with a favorable side-effect profile.
  • Penicillin V — classic first-line therapy, narrow spectrum, no known Group A Strep resistance.
  • Cephalexin (Keflex) — used for patients with mild penicillin sensitivity or when broader coverage is appropriate.
  • Azithromycin (Z-Pak) — reserved for penicillin allergy when cephalosporins are contraindicated. Note: Group A Strep resistance to macrolides exists; your physician will assess appropriateness.
  • Clindamycin — an option for patients with confirmed penicillin allergy and when macrolide resistance is a concern.

Most patients feel measurably better within 24–48 hours of starting antibiotics. Completing the full prescribed course is essential — stopping early increases the risk of treatment failure and complications.

What to Expect During Recovery

Alongside your antibiotic, your physician may recommend symptomatic relief measures for the 24-hour window before the antibiotic takes full effect:

  • Over-the-counter anti-inflammatory analgesics (ibuprofen or acetaminophen) for pain and fever reduction
  • Warm salt-water gargling for local throat relief
  • Increased fluid intake to prevent dehydration, especially with fever
  • Throat lozenges with anesthetic agents for temporary pain relief
  • Rest — your immune system does the heavy lifting alongside the antibiotic

Return to school or work: You are generally no longer contagious after 24 hours on antibiotics and once you are fever-free for at least 24 hours. Your physician will confirm the appropriate return-to-activity timeline based on your clinical response.

When It’s Not Strep — Most Cases Are Viral

The majority of sore throats seen in urgent care — an estimated 85–90% of adult cases — are caused by viruses. Antibiotics will not help a viral sore throat. Prescribing them unnecessarily contributes to antibiotic resistance and can cause side effects without benefit. This is why a rapid test matters.

Common Viral Causes

Rhinovirus, adenovirus, influenza, Epstein-Barr virus (mono), and COVID-19 are all common causes of severe sore throat that can mimic strep. Your physician will assess the clinical picture and test appropriately.

Symptomatic Treatment That Works

Viral sore throats resolve on their own in 7–10 days. Treatment focuses on comfort: anti-inflammatory analgesics for pain and fever, adequate hydration, rest, and throat lozenges. Your physician will advise on the safest options for your age and health history.

Complications of Untreated Strep Throat

Untreated strep throat is not simply uncomfortable — it carries a real risk of serious complications, particularly in children and adolescents. Antibiotics are prescribed not only to speed recovery but to prevent these sequelae.

Rheumatic Fever

An inflammatory condition affecting the heart, joints, skin, and nervous system. Can occur 2–4 weeks after untreated strep. In severe cases it leads to permanent heart valve damage (rheumatic heart disease). Rare in the US today but preventable with antibiotics.

Post-Streptococcal Glomerulonephritis

Kidney inflammation triggered by the immune response to Group A Strep. Can cause blood in urine, high blood pressure, and in rare cases kidney failure. Unlike rheumatic fever, antibiotics do not reliably prevent this complication but they reduce bacterial load and spread.

Peritonsillar Abscess

A collection of pus between the tonsil and the surrounding tissue. Presents with severe one-sided throat pain, difficulty swallowing, muffled voice (“hot potato voice”), and displacement of the uvula. Requires drainage and IV antibiotics — often an emergency department visit. Prompt strep treatment reduces this risk significantly.

Additional rare but recognized complications include Lemierre’s syndrome (thrombosis of the internal jugular vein), scarlet fever, and spread of infection to the middle ear (otitis media) or sinuses. None of these should be taken lightly. Treating confirmed strep with a simple antibiotic course is inexpensive, safe, and effective insurance against all of them.

Strep throat is bacterial, responds to antibiotics, and typically lacks runny nose or cough. Mono (infectious mononucleosis) is caused by the Epstein-Barr virus — it tends to cause profound fatigue, significant lymph node swelling, and splenomegaly. COVID-19 typically presents with additional respiratory symptoms, loss of smell, or taste changes. At TrufaMED, rapid testing for strep, influenza, and COVID-19 is available on the same visit when the clinical picture warrants it.

My child keeps getting strep. Is something wrong?

Recurring strep throat — typically defined as more than 3 confirmed episodes per year — is common in school-age children and warrants further evaluation. Potential factors include persistent household carrier status (someone in the family carrying strep without symptoms), tonsillar crypts that harbor bacteria, or immune factors. Your TrufaMED physician can evaluate patterns of recurrence, test household contacts when indicated, and discuss whether referral for tonsillectomy evaluation is appropriate. This is a common and manageable situation.

What if my rapid strep test is negative but I still feel terrible?

A negative rapid strep test does not automatically rule out strep in patients with strong clinical symptoms. TrufaMED physicians use a protocol consistent with IDSA guidelines — if your clinical presentation is highly suspicious despite a negative rapid test, a throat culture is sent to confirm. You will be contacted with results within 24 to 48 hours and treated appropriately if culture is positive. Your physician will also evaluate you for other causes of your symptoms including mono, influenza, and COVID-19.

Does TrufaMED test for mono (mononucleosis) as well?

Yes. A rapid mono test (monospot test) is available on-site at TrufaMED. If your physician suspects mono based on your symptoms — particularly significant fatigue, diffuse lymphadenopathy, or an unusually severe presentation — they can add this test to your visit. Mono requires different management than strep, including activity restrictions to protect the spleen.

However, treating with antibiotics is strongly recommended because: it speeds recovery by 1 to 2 days, it reduces contagiousness within 24 hours, and — most importantly — it prevents the serious complications of untreated strep, including rheumatic fever, kidney inflammation, and peritonsillar abscess. How do I know if someone in my household is a strep carrier?

A strep carrier is someone who tests positive for Group A Strep but has no symptoms — they are rarely contagious but can occasionally re-infect household members with recurrent strep. If your family experiences repeated strep episodes, your TrufaMED physician can evaluate household contacts with throat swabs to identify carrier status. Carriers may require a different antibiotic regimen to clear carriage.
