Joint Commission AccreditedA DOT physical is a federally mandated health check under 49 CFR Part 391.41. It confirms you meet the physical, mental, and emotional standards to safely operate a commercial motor vehicle. At TrufaMED you complete it in a single visit and walk out with your Medical Examiner’s Certificate in hand.
You need a current certificate if you operate a vehicle with a gross weight rating of 10,001 pounds or more, carry more than 8 passengers for compensation, carry more than 15 passengers regardless of compensation, or haul hazardous materials in placardable quantities. The whole visit usually runs about 30 minutes, and your result is uploaded to the FMCSA National Registry within 24 hours.
Every DOT physical follows the same FMCSA standard. Your examiner reviews your history, runs each required test, and explains the result before you leave.
A Snellen vision test for distant and peripheral sight in each eye and combined, plus a hearing check by forced whisper at 5 feet or an audiometric exam. Bring your glasses, contacts, or hearing aids.
Your blood pressure and pulse are measured and read against the FMCSA thresholds. Well-controlled hypertension is usually compatible with certification.
A simple urine sample is checked for protein, blood, sugar, and specific gravity. This is a screening step, not a drug test, though a DOT drug screen can be added.
A head-to-toe check of your cardiovascular, respiratory, neurological, musculoskeletal, and abdominal systems, performed under the direction of a board-certified physician.
A detailed review of your medical history and every prescription and over-the-counter medication you take. A complete, current list keeps the visit short.
If you pass, you receive your Medical Examiner’s Certificate (Form MCSA-5876) the same day, valid for up to 24 months, with electronic upload to the FMCSA registry.
Bring these items so your examiner can complete everything in a single appointment. Missing paperwork is the most common reason a certificate gets delayed. If you forget something we can usually still help, but arriving prepared keeps your visit short.
A driver’s license or any government-issued photo identification.
If you wear corrective lenses to drive, bring them. Vision is tested with whatever correction you use behind the wheel.

Most managed conditions are fully compatible with certification. The FMCSA standard is whether a condition is well-controlled and does not impair your ability to drive safely.
Controlled Stage 1 hypertension typically earns a one-year certificate. Stage 2 calls for treatment and a shorter, 3-month certificate, with re-certification once it is controlled. Severe, uncontrolled hypertension is disqualifying until it is brought under control.
Diabetes managed by diet or oral medication is generally not disqualifying. Insulin-treated diabetes requires the FMCSA Diabetes Assessment Form completed by your endocrinologist and follows the Insulin-Treated Diabetes Mellitus protocol.
A sleep apnea diagnosis is not automatically disqualifying. You demonstrate compliance with treatment, usually CPAP use of more than 4 hours a night on over 70 percent of nights. Suspected but undiagnosed apnea may require a sleep study before certification.
A history of heart attack, bypass, stenting, or a pacemaker requires a cardiologist clearance letter and may shorten the certification interval to a year or less. Implantable cardiac defibrillators are disqualifying.
Distant acuity must be at least 20/40 in each eye and 20/40 with both eyes, with or without correction, and your field of vision must be at least 70 degrees in each eye. For hearing, you must perceive a forced whisper at 5 feet or pass an audiometric exam.
A disqualification is rarely permanent. Your examiner explains exactly which finding flagged it, what evidence clears it, and the timeline. We can often schedule a prompt follow-up once you have the required clearance letters, and we provide full documentation to support an FMCSA Medical Review Board appeal if you choose to file one. The visit moves fastest when you bring everything we ask for: a photo ID, your current medication list, and any specialist clearance letters. Drivers with a managed condition, or who add a drug screen, breath alcohol test, or sleep apnea documentation, may need 45 to 60 minutes total.

Walk-ins are welcome seven days a week: Monday to Friday 9 AM to 9 PM, Saturday 11 AM to 11 PM, and Sunday 12 PM to 8 PM. We see DOT physicals throughout the day. If you want a specific time slot, you can register online in advance to skip the queue. Most walk-in patients are seen within about 25 minutes of arrival.
The standard Medical Examiner’s Certificate is valid for up to 24 months. A shorter certificate (12, 6, or 3 months) is issued when you have a managed condition that needs more frequent re-evaluation, such as well-controlled hypertension, diabetes, or sleep apnea. Your examiner explains your certification length and the reason for it before you leave.
Common reasons for immediate disqualification include insulin-treated diabetes without the proper FMCSA paperwork, a current epilepsy or seizure diagnosis, vision worse than 20/40 corrected in either eye, an inability to perceive a forced whisper at 5 feet without an audiometric pass, an implantable cardiac defibrillator, untreated severe sleep apnea, current narcotic use, and clinical alcohol or drug dependence. Many of these are temporary. Once the condition is treated, controlled, or properly documented, certification is often possible.
Yes. Every DOT exam result is uploaded to the FMCSA National Registry of Certified Medical Examiners within 24 hours of your visit, which is a federal requirement. You leave with a printed copy of your certificate, and you can verify your status on the FMCSA registry website using your driver’s license number.
A failed exam is rarely the end of your driving career. Your examiner explains exactly which finding triggered the result, what documentation or treatment is needed to qualify, and the likely timeline. Many drivers return to full certification within days or weeks after submitting a specialist clearance letter or completing a treatment program. We can often re-test once you have the required paperwork.
Yes. We complete your DOT physical and a DOT 5-panel drug screen in one visit, which is the most common request from drivers in pre-employment hiring or random testing. We also handle breath alcohol testing, school bus driver exams, and OSHA occupational physicals, so you do not have to make a second trip. Add-on services are billed separately from the physical.
