What is a Niagen IV?

Niagen is a patented, pharmaceutical-grade form of nicotinamide riboside chloride, a small vitamin B3 molecule that enters your cells directly and converts into NAD+, the coenzyme every mitochondrion uses to make energy. At TrufaMED, Niagen is given only as a 500 mg IV in our Surfside lounge under physician oversight. A session runs about sixty minutes, roughly three hours shorter than a comparable NAD+ IV, and for most patients without the flushing or chest pressure a direct NAD+ infusion can bring. Niagen IV is a wellness and longevity service, not a treatment for any disease.

Precursor, not NAD+ itself

Niagen feeds the NAD+ pathway; an NAD+ IV pours it in

Both raise cellular NAD+. The honest difference is the route. Knowing it is the easiest way to choose, and most TrufaMED longevity patients end up using both, each for what it does best.

NAD+ levels fall with age. By a person’s forties, tissue NAD+ has dropped well below the level of a twenty-year-old, a shift tied to lower mitochondrial efficiency and a weaker cellular stress response. There are two ways to address it.

A direct NAD+ IV works, but it works the hard way. NAD+ is a large molecule that does not cross the cell membrane easily and is partly broken down before it gets inside, which is why a full NAD+ IV runs three to four hours and is often paced to manage flushing or chest pressure. Niagen takes a different route. Nicotinamide riboside is small enough to pass through the cell wall directly, where your own enzymes (NRK1, NRK2, and NMNAT) convert it into NAD+ inside the cell. The pathway is already there. Niagen simply provides the substrate, which is why the session is shorter and, for most patients, easier to tolerate. Read more about our NAD+ IV.

Niagen 500 mg IV

The NAD+ precursor

Nicotinamide riboside, a small B3 molecule

  • Enters the cell directly through nucleoside transporters, then converts to NAD+ in place
  • About 60 minutes in the chair, roughly 75% shorter than a standard NAD+ IV
  • Clean tolerability: no attributable adverse events in the published 500 mg IV evaluation
  • Best for a monthly or biweekly longevity routine and a gentler first introduction

NAD+ IV

The NAD+ molecule itself

Direct coenzyme replenishment in circulation

  • Infused into the bloodstream, with limited membrane uptake and partial breakdown outside the cell
  • Three to four hours in the chair, paced to keep the rate comfortable
  • Can bring flushing, chest pressure, or nausea at faster infusion rates
  • Best for a periodic high-dose cellular reset or a quarterly anchor session
$1,000 per session

Three measured differences, not marketing

Published evaluation of a 500 mg Niagen IV against a conventional NAD+ IV points to three endpoints that matter to patients: higher NAD+ uptake, a shorter session, and a cleaner tolerability profile.

About 20% higher NAD+ at three hours

Clinical evaluation of a 500 mg IV Niagen dose measured intracellular NAD+ roughly 20% higher at the three-hour mark versus baseline, the endpoint most closely tied to the steady energy patients describe afterward.

Roughly 75% shorter in the chair

A full conventional NAD+ IV takes three to four hours to deliver safely. A 500 mg Niagen IV is complete in about an hour, a 75% reduction in chair time while keeping the NAD+ pathway benefit.

No attributable adverse events

The first clinical evaluation of intravenous nicotinamide riboside (ChromaDex, 500 mg IV) reported no attributable adverse events. In our experience, the most common report from a Niagen IV is the absence of any side effects at all.

Figures reflect published Niagen IV evaluation and TrufaMED clinical experience. Niagen IV is a wellness service; results vary and it is not a treatment for any disease.

Where cellular NAD+ does its work

Niagen is used in longevity medicine to support the physiology that depends on cellular NAD+: energy, repair, and healthy aging. Individual response varies.

Sustained cellular energy

NAD+ is the electron shuttle at the center of the mitochondrial electron transport chain. Raising intracellular NAD+ is the biochemical basis for the steady energy and stamina patients describe after a session.

Healthy aging and longevity

NAD+ powers the sirtuins (SIRT1, SIRT3, SIRT6) involved in cellular longevity signaling. Niagen is a common building block of evidence-informed longevity protocols alongside HBOT and periodic lab monitoring.

DNA repair and resilience

PARP enzymes consume NAD+ to repair DNA strand breaks after oxidative and UV damage. Keeping intracellular NAD+ available is one mechanism tied to cellular resilience under everyday metabolic stress.

Cognitive clarity and focus

Neuronal NAD+ status is involved in the mitochondrial energy that supports alertness and mental endurance. Executives and performance-minded patients often fold Niagen into a weekly or biweekly routine.

Metabolic support

NAD+ takes part in hundreds of enzymatic reactions, including lipid oxidation and glucose metabolism. Niagen is often paired with a physician-supervised metabolic and cardiovascular wellness plan.

Tissue repair and recovery

Stem-cell function, tissue regeneration, and inflammation resolution all depend on NAD+. Athletes and recovery-focused patients pair Niagen with amino acid IV therapy and HBOT.

Who benefits from a Niagen IV?

Niagen IV is used across a range of physician-supervised contexts, from high-output professionals to patients building a long-term longevity routine. Every candidate is screened for contraindications before a first session.

Longevity-focused adults

Patients building a structured healthy-aging routine, often in their 30s through 50s, use Niagen monthly or biweekly alongside lab monitoring to track metabolic and mitochondrial markers over time.

High-output executives

Founders and professionals under sustained cognitive and travel demand cycle Niagen for energy stability. Our concierge members fold it into quarterly performance protocols.

Athletes and recovery

High training volume depletes NAD+ stores. Niagen supports the cellular-repair pathways active in the recovery window, especially paired with hydration, amino IV therapy, and HBOT.

Patients already on NAD+

If you already know NAD+ IV, the difference is the experience: shorter sessions, no flushing, no chest pressure. Most keep a quarterly NAD+ IV as the anchor and add Niagen as monthly maintenance.

Peri- and post-menopausal patients

Energy metabolism and mitochondrial support are areas of active clinical interest during hormonal transitions. Coordinated with a physician’s overall plan, Niagen can be part of an energy and resilience strategy.

Post-illness and high-travel recovery

Patients recovering from a prolonged illness, heavy travel, or extended physical and cognitive demand use targeted NAD+-pathway support, always after a physician confirms it is appropriate.

Your Niagen IV session, start to finish

Every Niagen IV follows the same rhythm: physician-reviewed, nurse-administered, finished in about an hour.

Physician intake

First-time patients complete a brief physician intake covering medical history, medications, and goals, to confirm a 500 mg Niagen IV is the right fit. Return visits need only a shorter RN check-in.

IV placement

A registered nurse places a small-gauge catheter, usually at the inner elbow or forearm. Most patients feel a brief pinch, and placement takes under ninety seconds.

Infusion

Your 500 mg Niagen IV infuses over about sixty minutes in the lounge, with complimentary red light therapy alongside. Most patients work, read, or rest. No loading dose, no pacing pauses.

Discharge

The IV comes out, a brief check confirms you feel well, and your clinical record is updated. Most patients leave right away and return to a normal day, with the next session set on a physician-recommended cadence.

Dispensed only under physician standing orders

A Niagen IV at TrufaMED is given only under active physician standing orders reviewed by our Medical Director. Your session is never unsupervised and never administered by anyone outside the clinical team.

Precautions and contraindications

A Niagen IV is given only after a physician-supervised intake. These are the standard precautions every patient is screened against before a first session.

Do not get a Niagen IV if you

Not appropriate

  • Have a current or prior history of cancer (NAD+ pathway interactions with cellular proliferation require physician clearance)
  • Are pregnant or breastfeeding, since Niagen IV has not been evaluated in these groups
  • Have active or uncontrolled rheumatoid arthritis (physician review required)
  • Have a known hypersensitivity to nicotinamide riboside or any component of the formulation
Talk to your physician if you

Review first

  • Take diabetes medications, since Niagen may influence glucose metabolism; monitor blood sugar
  • Are on blood thinners such as warfarin, DOACs, or antiplatelets, where added site care may apply
  • Have chronic kidney or liver disease, where dosing is individualized to organ function
  • Take other NAD+ precursors or high-dose B3 supplements, so timing can be coordinated

This list is not exhaustive. A complete medical history and medication list are reviewed before every first session. Niagen is a compounded injectable drug and is not FDA-approved as a therapeutic. Adverse events are reported to fda.gov/medwatch. For a medical emergency, call 911.

Niagen IV, answered

What patients across Miami ask most before booking a Niagen session.

What is a Niagen IV?

Niagen is a patented, pharmaceutical-grade form of nicotinamide riboside chloride, a vitamin B3 molecule that acts as a direct precursor to NAD+. At TrufaMED it is given exclusively as a 500 mg IV over about sixty minutes, delivered by a registered nurse in our Surfside lounge under physician oversight. Niagen IV is a wellness and longevity service, not a treatment for any disease.

How is a Niagen IV different from an NAD+ IV?

An NAD+ IV delivers the NAD+ molecule itself, a large coenzyme that has to be infused slowly over three to four hours and can bring flushing or chest pressure. A Niagen IV delivers nicotinamide riboside, a smaller B3 molecule that passes through the cell wall directly and is converted to NAD+ inside the cell. Published clinical evaluation of a 500 mg IV Niagen dose measured roughly 20% higher NAD+ at three hours versus baseline, with no attributable adverse events, in a session of about 60 minutes. Niagen does not replace NAD+ IV; the two work through different mechanisms and we offer both.

Why only 500 mg? Can I get a lower dose?

No. TrufaMED administers Niagen exclusively at 500 mg and does not offer a reduced-dose tier. The clinical evaluation that supports Niagen IV used the 500 mg dose, and we keep every session at full strength rather than offering a partial one. If 500 mg Niagen is not the right fit, your physician may recommend our NAD+ IV program instead.

How long does a Niagen IV session take?

A 500 mg Niagen IV runs about sixty minutes including setup, roughly 75% shorter than a standard NAD+ IV, which typically takes 180 to 240 minutes. Most patients work, read, or rest during the infusion, with complimentary red light therapy running alongside.

What are the side effects of Niagen IV?

The first clinical evaluation of a 500 mg IV Niagen dose reported no attributable adverse events. In our experience most patients have no flushing, no chest pressure, and no nausea, which are the three discomforts most often reported on a conventional NAD+ IV. Occasional mild warmth at the infusion site can occur. Niagen IV is not appropriate for patients with current or prior cancer, pregnancy, breastfeeding, active rheumatoid arthritis, or a known hypersensitivity, all of which are screened during intake.

How often can I get a Niagen IV?

A typical longevity-maintenance cadence is monthly to every other week. Most TrufaMED patients settle into one or two sessions a month. Your physician recommends a schedule after your intake based on your goals, your response, and biomarker monitoring. Our package of four is built around a quarterly arc, usually four sessions spaced two to four weeks apart.

How much does a Niagen IV cost at TrufaMED?

Niagen is sold as a package of four 500 mg sessions for $3,200, which works out to $800 per session. A single session is $1,200. Niagen IV is also included as part of the TrufaMED Concierge Signature membership. It is a self-pay wellness service and is not covered by insurance. We accept major credit cards, FSA, and HSA.

Do you still offer NAD+ IV?

Yes. NAD+ IV remains on the TrufaMED longevity menu at $1,000 per session in-clinic. Niagen IV is our newer flagship NAD+-pathway protocol, but a direct NAD+ IV stays available for patients who specifically want direct NAD+ replenishment or who prefer to combine both therapies in sequence. Your physician helps design the protocol that fits your goals.

Is Niagen FDA-approved?

Niagen injectable is a compounded drug prepared by a licensed US compounding pharmacy and is not an FDA-approved therapeutic. It is dispensed for office use only under an active physician standing order. Nicotinamide riboside as a raw ingredient is a generally recognized safe dietary form of vitamin B3. Adverse events are reported to fda.gov/medwatch.

Is a Niagen IV available at home or my hotel?

Yes. The 500 mg Niagen IV is available in our Surfside lounge and as a mobile registered-nurse visit across South Florida, including Miami Beach, Bal Harbour, Sunny Isles Beach, Bay Harbor Islands, Aventura, and Brickell. Book in-clinic online, or arrange a mobile visit through our mobile IV service.

When will I feel the effects?

Most patients describe a clean, steady lift in energy and clarity during the infusion and for hours afterward. The biochemical endpoint cited in clinical evaluation, about 20% higher NAD+ versus baseline, peaks near the three-hour mark after infusion. The cumulative longevity benefit builds over a consistent monthly or biweekly cadence, which is why most patients choose the package of four. Individual response varies.

Who should not get a Niagen IV?

Niagen IV is administered only after a physician-supervised intake. It is avoided in patients with a current or prior history of cancer, pregnancy or breastfeeding, active rheumatoid arthritis, or a known hypersensitivity to nicotinamide riboside. Patients on diabetes medications, blood thinners, or with chronic kidney or liver disease are reviewed individually. A complete history and medication list are checked before every first session.
