Yes. TrufaMED performs sports, school, camp, DOT, and work physicals on a walk-in basis, seven days a week. Most physicals take 20–30 minutes and the physician signs and returns your form at the visit. Bring your form, ID, insurance card (if using insurance), and a list of current medications. Self-pay is $200. The only exception is the Immigration (USCIS I-693) physical, which requires a scheduled visit at $325.
TrufaMED handles the full menu of routine physicals at our Surfside clinic. Every exam is performed and signed by a board-certified physician, and most forms are completed at the visit so you don’t leave empty-handed.
Preparticipation physical examination for youth and high school athletes, cheer, dance, and scouts. We complete the Florida High School Athletic Association (FHSAA) EL2/EL3 form, AAP Preparticipation forms, and most school district forms.
Summer camp and sleepaway camp health forms. Allergy and medication documentation, immunization verification, and camp-specific clearance signed the same day.
Commercial driver license medical examination by a Federal Motor Carrier Safety Administration certified provider. Medical Examiner’s Certificate (MEC 649-F) issued on-site when you pass.
Employer-required physicals, including lifting-capacity, vision, and hearing screening. Urine drug screen available as an add-on. Paperwork returned at the visit.
Preventive exam covering vitals, cardiovascular risk screening, routine lab orders, and age-appropriate cancer screening guidance. Billed to insurance as a preventive visit.
Civil surgeon examination for green card applicants. By appointment only. Tuberculosis screening, required labs, vaccine review, and sealed I-693 form completion.
Whether it’s a sports form or a DOT medical, the physician-led exam covers the same standard checkpoints. Additional screening for specific physicals (hearing, vision, urine dip, drug screen) is added as required by the form type.
Blood pressure, heart rate, respiratory rate, temperature, height, weight and BMI. Documented on your form.
Auscultation of heart and lungs, pulse and rhythm assessment, screening questions for family cardiac history and exertional symptoms.
Range-of-motion testing, joint stability screening, spine and posture assessment — especially important for PPE sports physicals.
Snellen visual acuity and screening audiometry when required by the form (e.g. DOT, certain schools, employer forms).
Cranial nerve, reflex, and coordination screen. Abdominal exam and hernia check where indicated.
Your form is signed, stamped, and returned at the visit by a board-certified physician, including any notes on clearances or restrictions.
Bring the blank form from your school / employer / camp, a photo ID, and insurance card if billing. Parents or guardians should accompany minors.

A board-certified physician performs your full physical: vitals, heart & lung exam, musculoskeletal screen, vision / hearing as required.
Your form is completed, signed, and returned at the visit. Copies kept in your TrufaMED chart for easy future access.

No. Sports, school, camp, DOT, and work physicals are walk-in, seven days a week. Arrive any time we’re open and we’ll get you seen. Only the Immigration (USCIS I-693) physical requires a scheduled appointment.
Most physicals take 20–30 minutes from check-in to leaving with a signed form. DOT and adult annual wellness visits may take 30–45 minutes. Total time, including any wait, is usually under an hour.
Self-pay for sports, school, camp, DOT, and work physicals is $200. The Immigration I-693 physical is $325. If you have insurance, we’ll bill your adult annual wellness as a preventive visit. Call (305) 537-6396 and we’ll verify your benefits in under five minutes.
Bring the blank form from your school, employer, camp, or FHSAA, a photo ID, and your insurance card if you’re billing. Minors must be accompanied by a parent or guardian. A current medication list and immunization record help us complete the form accurately.
Yes. TrufaMED has National Registry Certified Medical Examiners on staff. We issue the Medical Examiner’s Certificate (MEC Form MCSA-5876) on-site if you pass the examination. Bring your current CDL and a list of current medications.
Yes. Our physicians complete and sign the FHSAA preparticipation forms (EL2/EL3), AAP preparticipation forms, and most Miami-Dade school district health forms at the visit.
Yes. We stock and administer most pediatric school-required vaccines (DTaP/Tdap, MMR, hepatitis A/B, varicella, meningococcal, HPV) and can update your Florida 680 immunization form at the visit.
