The exam that clears your child to play
A sports physical, formally a preparticipation physical evaluation (PPE), confirms it is safe for a student athlete to train and compete. At TrufaMED a board-certified physician runs the exam, completes the school or league form, and hands it back signed before you leave. Most visits take 20 to 30 minutes, and you can walk in any day we are open.
Bring the blank form from your school, league, or the Florida High School Athletic Association, a photo ID, and your insurance card if you are billing. We complete the FHSAA EL2 and EL3 forms, the AAP preparticipation forms, and most Miami-Dade school district forms. Self-pay is a flat $200, with no surprise add-on fees for a routine exam.
A thorough physical, done in one visit
Every sports physical follows the same standard checkpoints. Your physician reviews the history, runs each screen, and notes any clearance or restriction directly on the form.
Medical history
A review of past injuries, surgeries, asthma, allergies, current medications, and the family history questions that matter most for young athletes, including any history of fainting or early cardiac death.
Vital signs and BMI
Blood pressure, heart rate, respiratory rate, temperature, height, and weight, with body mass index documented on the form. Elevated readings are explained, not just recorded.
Cardiovascular screen
The physician listens to the heart and lungs, checks pulse and rhythm, and asks the standardized screening questions about chest pain, fainting, and palpitations with exertion. This is the part that matters most for athletes.
Musculoskeletal exam
Range-of-motion testing, joint stability screening, and a spine and posture assessment. Old injuries are checked for full recovery before clearance to return to play.
Vision and hearing
Snellen visual acuity and, when a form requires it, a screening hearing check. Bring glasses or contacts if your child wears them to play.
Neurologic and abdominal
A brief reflex and coordination screen, plus an abdominal exam where indicated. Concussion history is reviewed before sign-off for contact sports.
A sports physical is not a full annual checkup
The preparticipation exam is focused on one question: is it safe for your child to play? It covers history, vitals, the heart and lung screen, and a musculoskeletal check. It does not replace a complete annual wellness visit, which covers growth, development, full immunization review, and broader preventive care. Many families do both in the same visit. No appointment needed for sports, school, camp, or work physicals. Open Monday to Friday 9 AM to 9 PM, Saturday 11 AM to 11 PM, and Sunday 12 PM to 8 PM.

A board-certified physician performs the full physical: history, vitals, the heart and lung screen, the musculoskeletal check, and vision or hearing as the form requires.
Leave with the signed form
Your form is completed, signed, and returned at the visit. A copy stays in your TrufaMED chart so next season’s form is even faster.
A board-certified physician signs every exam
Every physical at TrufaMED is performed and signed by a board-certified physician. Our Chief Medical Officer and Medical Director set the clinic’s examination protocols, so clearance is consistent, documented, and defensible.
Dr. Gedalia leads TrufaMED’s clinical strategy. His surgical and acute-care background informs every examination protocol at the clinic, from cardiovascular screening standards to musculoskeletal clearance for return to play.
Dr. Naidoo oversees day-to-day clinical operations and supervises every urgent care shift. He has extensive experience completing sports, school, and work physicals for patients of all ages, with calm, decisive judgment on every clearance.
TrufaMED is Joint Commission accredited, a distinction held by only eight urgent care centers in the United States and the only one in Florida. It is the same standard applied to major hospitals, and it governs every exam we perform.

Do I need an appointment for a sports or school physical?
No. Sports, school, and camp physicals are walk-in, seven days a week. Arrive any time we are open and we will get your student athlete seen. Most families are in and out in about half an hour. The only physical that requires a scheduled appointment is the immigration (USCIS I-693) exam, which is handled separately.
How long does a sports physical take at TrufaMED?
Most sports and school physicals take 20 to 30 minutes from check-in to walking out with a signed form. Total time, including any short wait, is usually under an hour. Coming prepared with the blank form and a current medication list keeps the visit moving.
How much does a sports physical cost?
Self-pay for sports, school, and camp physicals is a flat $200, with no surprise add-on fees for a routine exam. If you are billing the visit as part of an adult annual wellness exam, we can bill most insurance. Call (305) 537-6396 and we will verify your benefits in a few minutes.
What should my child bring to a sports physical?
Bring the blank form from the school, league, or the Florida High School Athletic Association, a photo ID, and your insurance card if you are billing. A minor must be accompanied by a parent or guardian. A current medication list and immunization record help the physician complete the form accurately.
Can you sign the Florida FHSAA (EL2 and EL3) preparticipation form?
Yes. Our physicians complete and sign the FHSAA preparticipation forms (EL2 and EL3), the AAP preparticipation forms, and most Miami-Dade school district health forms at the visit. You leave with the completed paperwork the same day, signed by a board-certified physician.
Is a sports physical the same as a full annual checkup?
No, and it is important to know the difference. A sports physical (a preparticipation evaluation) is focused on whether it is safe for your child to play: history, vitals, a heart and lung check, and a musculoskeletal screen. It does not replace a complete annual wellness visit with your pediatrician, which covers growth, development, immunizations, and broader preventive care. We are glad to do both.
Why does the heart screening matter for young athletes?
Sudden cardiac events in young athletes are rare but serious, and the preparticipation exam is the main chance to catch a warning sign early. Our physicians listen to the heart and lungs, check pulse and rhythm, and ask the standardized screening questions about fainting, chest pain with exertion, and family history of early cardiac death. Anything concerning is flagged before clearance, with a referral when warranted.
Do you also complete summer camp physicals?
Yes. We complete summer camp and sleepaway camp health forms, including allergy and medication documentation, immunization verification, and camp-specific clearance, signed the same day. Camp physicals follow the same walk-in, single-visit process as school sports physicals.
My child needs vaccines before school. Can you handle those at the same visit?
Yes. We stock and administer most pediatric school-required vaccines, including Tdap, MMR, hepatitis A and B, varicella, meningococcal, and HPV, and we can update your Florida 680 immunization form at the visit. Bringing the existing immunization record helps us record everything correctly.
