Understanding the flu

What the flu is, and who it hits hardest

Influenza is a respiratory virus that comes on fast. Influenza A is the most common and usually the more severe type, responsible for most seasonal epidemics. Influenza B also circulates each season and tends to run a little milder. What people call the stomach flu is something different: that is gastroenteritis, a stomach and intestinal illness, not true influenza, though we treat that too. Caught early, the flu can be treated with an antiviral such as oseltamivir (Tamiflu) to shorten how long you are sick and ease how severe it gets, which is why timing matters. Started later, the flu is managed with supportive care: rest, fluids, and steady fever and pain control. One thing we are always direct about is that antibiotics do not treat the flu. The flu is a virus, and antibiotics only work on bacteria. If a bacterial complication develops, such as a sinus infection or pneumonia, your physician treats that on its own.

Who should be seen promptly

For most healthy adults the flu is miserable but safe to ride out with rest and fluids. Some people are at higher risk of it turning serious and should be evaluated early rather than waiting: adults 65 and older, children under 5 (especially under 2), pregnant patients, and anyone with a chronic condition such as asthma, COPD, heart disease, or diabetes, or a weakened immune system. Honest about the warning signs

When the flu becomes an emergency

The flu is usually safe to treat at urgent care. A small number of symptoms mean you should skip the wait and get emergency care right away. If you notice any of these, call 911 or go to the nearest emergency room.

Trouble breathing

Shortness of breath, labored or fast breathing, or chest pain or pressure. In children, watch for fast breathing, flaring nostrils, or ribs pulling in with each breath.

Severe dehydration

Persistent vomiting, dizziness when standing, little or no urination, or in children no tears when crying and a dry mouth. Severe fluid loss needs prompt care.

Confusion or won’t wake

Sudden confusion, a child who is unusually sleepy or hard to wake, seizures, or bluish lips or face. These are emergencies, not flu symptoms to wait out.

Fever that won’t break

A very high fever that will not come down, or a fever that returns after it had improved. A returning fever can signal a secondary infection such as pneumonia.

How do I know if I have the flu, a cold, or COVID?

Do I need Tamiflu for the flu?

Not always. Many healthy adults recover with rest, fluids, and medication for fever and aches. How long does the flu last?

Most people start to feel better within about 3 to 5 days, though a lingering cough and fatigue can last a week or two. If you are not improving after a few days, your fever returns after it had broken, or you develop chest pain or trouble breathing, come back for a re-evaluation. Those can be signs of a secondary infection such as pneumonia.
