Whether it’s a sports form or a DOT medical, the physician-led exam covers the same standard checkpoints. Additional screening for specific physicals (hearing, vision, urine dip, drug screen) is added as required by the form type.

Auscultation of heart and lungs, pulse and rhythm assessment, screening questions for family cardiac history and exertional symptoms.
Range-of-motion testing, joint stability screening, spine and posture assessment — especially important for PPE sports physicals.
Snellen visual acuity and screening audiometry when required by the form (e.g. DOT, certain schools, employer forms).
Cranial nerve, reflex, and coordination screen. Abdominal exam and hernia check where indicated.

Do I need an appointment for a sports or school physical?
No. Sports, school, camp, DOT, and work physicals are walk-in, seven days a week. Arrive any time we’re open and we’ll get you seen. Only the Immigration (USCIS I-693) physical requires a scheduled appointment.
How long does a physical take?
Most physicals take 20–30 minutes from check-in to leaving with a signed form. DOT and adult annual wellness visits may take 30–45 minutes. Total time, including any wait, is usually under an hour.
How much does a physical cost?
Self-pay for sports, school, camp, DOT, and work physicals is $200. The Immigration I-693 physical is $325. If you have insurance, we’ll bill your adult annual wellness as a preventive visit. Call (305) 537-6396 and we’ll verify your benefits in under five minutes.
What do I need to bring?
Bring the blank form from your school, employer, camp, or FHSAA, a photo ID, and your insurance card if you’re billing. Minors must be accompanied by a parent or guardian. A current medication list and immunization record help us complete the form accurately.
Do you perform DOT (CDL) physicals?
Yes. TrufaMED has National Registry Certified Medical Examiners on staff. We issue the Medical Examiner’s Certificate (MEC Form MCSA-5876) on-site if you pass the examination. Bring your current CDL and a list of current medications.
Can you sign my Florida FHSAA (EL2/EL3) form?
Yes. Our physicians complete and sign the FHSAA preparticipation forms (EL2/EL3), AAP preparticipation forms, and most Miami-Dade school district health forms at the visit.
My child needs vaccines before school — can you do those too?
Yes. We stock and administer most pediatric school-required vaccines (DTaP/Tdap, MMR, hepatitis A/B, varicella, meningococcal, HPV) and can update your Florida 680 immunization form at the visit.
