TrufaMED physicians coordinate travel to Fisher Island by private ferry or by arrangement with the member’s preferred access method. Standard transit time is approximately 25 minutes from our Surfside clinic by car and ferry. For Signature members, scheduling is handled directly through your physician’s personal line. For yacht berth visits, a minimum of four hours advance notice is required for safety and logistics coordination.

Signature membership begins at $5,000 per month for family coverage, or $54,000 annually with priority scheduling guaranteed. It covers unlimited physician access for all primary household members, in-residence house calls, yacht visits, 24/7 telehealth, comprehensive annual physicals, wellness labs, concierge pharmacy coordination, specialist liaison, international travel medicine, and quarterly comprehensive health reviews. Additional residences can be accommodated by arrangement.

Yes. Signature membership accommodates seasonal residency patterns. Many Fisher Island residents maintain the membership year-round for continuity of physician relationship and access during Miami season (October through May). Your physician retains your medical history and family health context regardless of season. Telehealth remains available when you are outside Miami.

TrufaMED’s clinical team collectively speaks more than eight languages — including Russian, Portuguese, Spanish, French, Italian, Hebrew, and English. For Fisher Island’s international resident community, care in a patient’s first language is a clinical priority, not a secondary accommodation. Please indicate your language preference at enrollment and we will ensure your primary physician match is appropriate.

Yes. Coordination with out-of-city and international specialists is a core function of the Signature membership. Your TrufaMED physician reviews specialist notes, manages pre-appointment lab work, follows up on recommendations, and ensures continuity between providers. If you have established relationships with physicians in New York, Los Angeles, London, or elsewhere, TrufaMED functions as the integrating physician for your Miami presence — not a replacement for those relationships.

Every TrufaMED physician visit to Fisher Island or a vessel includes a clinical kit with a fully equipped diagnostic set — stethoscope, blood pressure, pulse oximetry, 12-lead ECG capability, point-of-care rapid testing for common infections, glucometer, and a medication kit appropriate for the visit type. IV administration equipment is available for Signature members who incorporate IV therapy into their care plan. Advanced diagnostics requiring imaging are coordinated at the clinic same-day or next-day.

TrufaMED is physician-led at every level of the concierge program. Signature members are enrolled with a specific physician — Dr. Gedalia or Dr. Naidoo — who serves as their primary physician. Urgent calls and acute consultations go to that physician directly, not to a nurse line or on-call staff. This is a non-negotiable design principle of the practice. No mid-level providers substitute for physician access in the Signature tier.

All patient relationships at TrufaMED are protected under HIPAA and physician confidentiality obligations. For Signature members with public profiles, additional protocols are available upon enrollment — including non-public appointment scheduling, discrete billing arrangements, and documented confidentiality agreements between the practice and household staff with clinical access. Patient names, visit frequency, and health information are never discussed outside the physician-patient relationship.

Concierge membership fees are self-pay and are not billed to insurance. Many Signature members use HSA or FSA accounts for eligible services. For specific medical services rendered during in-office visits — labs, imaging, procedures — insurance can be billed through TrufaMED’s CLIA-certified facility for covered members. Accepted carriers include Aetna, Cigna, United Healthcare, Humana, Oscar Health*, and Medicare. We do not accept or at this time.

The defining differences are access and panel size. Standard primary care physicians carry patient panels of 1,500 to 2,500 patients. TrufaMED’s Signature panel is a fraction of that — allowing genuine availability, extended appointment time, and a physician who knows your history without reviewing notes in the room. Concierge medicine also eliminates the scheduling friction — no referrals required for same-day visits, no waiting weeks for an appointment, no rotating provider pool. You have one physician. They are available.

Yes. Many Signature members add household staff coverage as part of their membership arrangement. Domestic staff, security personnel, and estate management teams can be covered under a separate enrollment plan. This is handled as a discrete arrangement and can be structured to maintain appropriate privacy boundaries between employer and staff medical information.
