What Florida requires for school entry
Form DH 3040, for first-time entry into a Florida school
Florida requires a school entry health exam for every student entering a Florida school for the first time, kindergarten included. The exam has to be dated within 12 months of the first day of school, and it is recorded on state form DH 3040. This is the form the school asks for before your child can start.
A school sport requires the sports physical at $200
The $125 school physical covers form DH 3040. It does not include an EKG and it does not include the FHSAA forms. A student who tries out for, practices with, or competes on a Florida high school team needs the sports physical instead, which is $200 and includes the EL1 EKG plus the EL1, EL2, and EL3 forms completed by our physicians. Bring the school’s blank DH 3040 form and your child’s immunization record.
A parent comes along
A parent or legal guardian has to accompany any patient under 18 and sign the consent. Bring a photo ID for yourself.

Physician-led exam
A board-certified physician performs the exam, completes the form, and signs it before you leave. A copy stays in your child’s chart.
School vaccines we stock in Surfside
We give these in the clinic and record them in your child’s chart. Bring the existing immunization record so nothing is repeated.
Tdap
Tetanus, diphtheria, and whooping cough. $155.
MMR
Measles, mumps, and rubella, for a student missing a documented dose. $130.
Varicella
Chickenpox vaccine, for a student without documented immunity. $190.
Hepatitis B
$105 per dose, common for a student catching up on the childhood series.
Influenza
$50 during flu season, given at the same visit as the exam.

Do we need an appointment for a school physical?
Yes. School physicals are by appointment. Call (305) 537-6396 to schedule. A parent or legal guardian has to come with any patient under 18 and sign the consent.
How much does the school physical cost?
The DH 3040 school physical is $125 self-pay. That price runs through October 1, 2026, and it is self-pay only, never billed to insurance.
Who has to have the DH 3040 exam?
Every student entering a Florida school for the first time, kindergarten included. The exam has to be dated within 12 months of the first day of school, and the school keeps the completed DH 3040 on file.
Does the $125 school physical include an EKG?
No. A DH 3040 school entry exam does not require an EKG, so the $125 exam does not include one. The EKG belongs to the FHSAA athletic lane, which is the separate sports physical exam described above.
What should we bring to the visit?
The school’s blank DH 3040 form, your child’s immunization record, and a photo ID for the parent or guardian. A current medication list helps the physician complete the form accurately.
