Severe migraine and tension headache that has not responded to your home medication regimen warrants same-day evaluation. Same-day care is especially valuable for: a migraine that has lasted longer than 4 hours despite oral medication, severe nausea preventing oral medication uptake, a headache that woke you from sleep, the first severe headache of your life, sudden-onset "thunderclap" headache (this is an ER call, not urgent care), or any headache with new neurological symptoms (vision changes, weakness, confusion).
TrufaMED's acute migraine protocol typically combines IV magnesium sulfate (which downregulates the NMDA receptor pathway driving migraine pain), IV ketorolac (Toradol — a non-opioid anti-inflammatory that controls inflammation-mediated head pain), IV anti-nausea medication (Zofran or Reglan), and a 1-liter saline carrier. Most patients report meaningful relief within 30 to 45 minutes of infusion start. The full visit takes 60 to 75 minutes including initial evaluation.
For patients with frequent migraines (more than 4 attacks per month), TrufaMED can develop a longer-term prevention strategy — including daily preventive medications, lifestyle modification (sleep, hydration, identified trigger avoidance), and regularly-scheduled monthly Myers' Cocktail or magnesium-focused IV therapy. Concierge members get integrated migraine management as part of their plan; non-members can transition to the program after the acute visit.
For sudden "worst headache of life," headache after head trauma, headache with stiff neck and fever, headache with confusion, weakness, vision loss, or trouble speaking, or headache in a pregnant patient with high blood pressure — go to the ER, not urgent care. These can signal stroke, meningitis, hemorrhage, or pre-eclampsia.
